| Insurance At Osborne Village Dental we accept most insurance plans. Claims are submitted electronically for faster processing. You must provide us with all information on your insurance card, along with your current address and date of birth. Please bring your card with you to your first appointment. If you have dual insurance, we require the above mentioned information for both plans. Payments are collected directly from insurance companies. If you do not have full coverage, you are required to pay your portion at each appointment. If your insurance is non-assignable you are required to pay for treatment in full at the time of service. We will then submit the insurance claim on your behalf and the insurance company will send you a cheque. Insurance policies are a contract between you and your company. Therefore, it is your responsibility to report any changes in your insurance plan or personal information to our receptionists immediately. Predetermination for major treatment is also provided free of charge. We accept Visa, MasterCard, Interac or cash. |
